粉瘤/epidermal cyst/atheroma
粉瘤とは良性の皮膚腫瘍で､皮膚科の日常診療ではよく診断切除される袋状のできものです｡粉瘤は皮膚の下にでき、老廃物がたまったもので、アテロームや表皮嚢腫と呼ばれることもあります。粉瘤は良性の腫瘍であり､多くの場合そのままにしておいても問題はありませんが､粉瘤が大きくなってきて､しこりのように感じたり､強い臭いを伴うようになるなど日常生活に支障をきたすようになってくる場合があります｡また粉瘤に細菌感染症が起こると､炎症を起こして化膿して強い痛みや腫れを生じるようなことがあります｡この場合には緊急的に膿を出す処置とともに､粉瘤をくりぬいてしまうことが必要になります｡

粉瘤はニキビと判断されることも多いですが､粉瘤とニキビは実は全くの別物です｡ただし､炎症の強いニキビは粉瘤と鑑別が難しい場合もあるため､まずは当院や近くの皮膚科を受診し判断してもらうことをおすすめいたします｡粉瘤と診断された場合には､完全に除去してしまうことも検討ください｡粉瘤はニキビとは違って､自然に無くなってしまうことはないからです｡当院の大阪梅田院･大阪心斎橋院･名古屋院･福岡院では粉瘤のくりぬき法を専門におこなっており､当日手術も可能です｡粉瘤のくりぬき法の手術は多くの場合5分から10分程度です｡

炎症性粉瘤/ epidermal cyst/atheroma with inflammation
小さい粉瘤では多くの場合､日常診療では支障をきたしません｡しかし､粉瘤が炎症を起こした場合には､小さい粉瘤であっても緊急の治療が必要になります｡粉瘤が炎症を起こしたものを炎症性粉瘤と皮膚科では呼びますが､その粉瘤の炎症を起こす原因は細菌感染と異物反応と考えられています｡粉瘤の中には老廃物がたくさん溜まっていますが､それらの中で細菌が急激に増殖した場合､感染症の状態となって炎症性粉瘤となります｡また粉瘤の中身が､粉瘤の外側になにかしらの刺激をきっかけに漏れ出てしまった場合に､異物反応がおきて､炎症性粉瘤となってしまう場合があります｡細菌感染症による炎症性粉瘤は夏期に多い印象で､汗などで皮膚が蒸れる時期に多く､異物反応による炎症性粉瘤は冬など乾燥でかゆみが増す時期に多い印象です｡
通常は小さな粉瘤でも､炎症性粉瘤となった場合に､痛みを伴った症状を起こします。局所の炎症のみであれば粉瘤が命に関わるような状況にはなりえませんが､炎症性粉瘤をそのまま放っておくと、粉瘤の炎症が周囲にも広がって､さらには全身に広がって発熱したりする場合もでてきます｡その場合には､粉瘤とはいえ入院となるような場合もあり得ます｡また粉瘤の炎症が長引くと、色素沈着や瘢痕などのリスクがあがりますので､整容的な意味でも炎症性粉瘤にはすみやかにくりぬき法をすることをおすすめいたします｡
