This patient has struggled with infected cysts throughout his life. You can appreciate some scar tissue behind his ear and should always approach these posteriorly for cosmetic reasons. This healed fairly quickly.

This accumulated quickly for this patient likely secondary to an infected hair follicle. I mistakenly mention "cavity" instead of "capsule" when I'm differentiating between an abscess and an infected cyst. The latter has a capsule where the former does not. Also, at the end of this procedure, I also sent off a swab but I stopped the video before I did that step. This was obviously not sutured closed and was not packed either.

I have had this zit/pimple my entire life! It never fully heals and it is always leaking this white pimple like pus. Skin will grow over it but it will bust easily. I have been tested for everything I can think of and all things are negative. It never gets better or worse. Doctors don't know the answer. What do you think it is?

This patient had a recently inflamed, infected cyst on the back of the neck. He was placed on oral antibiotics the week prior, and scheduled today for excision.
When I saw him and felt the bump, it was boggy and fluctuant, meaning that it felt soft and filled with fluid within, so I suspected there was still some inflammation in the area, and that I probably shouldn't excise the lesion at this time.
If I were to attempt excision at the time of this visit, I would have had to remove the entire bump there, which is just a little smaller than a golf ball, so the resultant excision line and scar would have been very long, potentially the entire width of the back of his neck!
Here, I do an I & D (an incision and drainage) or a cyst that has already drained to a great degree a few days prior. As you can see, there are fragments of the cyst wall and inflammation within this bump.
He will continue on his antibiotics, and in the future, if the cyst recurs, we can schedule an excision when it is not irritated and inflamed. In doing so, the scar line will be shorter, and there is less risk of spread of bacterial infection.

Christmas doesn't come every day!!This gentleman came to see me just a couple weeks ago, referred by a internist because he has a skin cancer, a basal cell carcinoma, on his chest that he needs to have removed completely. He is quite a character, and I couldn't help but notice that he has a lot of sun damage, and many comedones (blackheads and whiteheads). He used to be a fireman (I think in Indiana), one of his many jobs in his life, and he was raised on a farm. The years of sun exposure and also exposure to soot from fires, have likely caused him to develop quite a few blackheads and whiteheads. He doesn't like them, and has tried to squeeze them out, but hasn't even very successful.As we were finishing up our visit, I decided I would ask him whether he knew he had a lot of blackheads on his face, and whether he would like me to remove them? His response was great: "I would walk through a wall of fire if you could do that for me!" Always amazes me how interested my patients are in me getting rid of such things. Makes it all worthwhile! I realize I didn't extract all of his blackheads, but he is a work in progress! We will be seeing him at least two more times and will do further blackhead extractions.
