The patient finally came back for extractions of super soft and crumbly blackheads on the back. These numerous dilated pore of Winer are older than me and they had the consistency of clay. I DONT use metal extractors on these huge pores because they crush the delicate vascular plexus. Don't fret, you will see the extractor in action on the smaller ones in parts 2 and 3 of this series.First of all, I want to thank you for your comments. I do appreciate the interest. Although I am a board-certified dermatologist and fellowship-trained micrographic and reconstructive surgeon, I will honestly say that “pimple popping” is nowhere in our educational curriculum. Believe it or not, it is a term used by other doctors to disparage our field and cast shame on the insignificance of what we do. I believe that is why viewers may also believe that you possess more knowledge on this subject than a physician who holds the title of a board-certified dermatologist. With regard to the use of comedones extractors on the back of an old gentleman with Dilated Pore of Weiners that have the consistency of wet clay; the force required to express these very large, super soft/wet keratin contents in super deep pores on 1cm thick back skin was so high that it was crushing the blood vessels in the dermal plexus. If there is any chance for me to be able to sew these up at a later date so they don't fill again, I need an intact subdermal plexus. I hope that explains things.
